Fagen Pharmacy values our relationship with our customers and respects their right of privacy. Any private personal information obtained, such as names, addresses, telephone numbers, social security numbers, private health care information, and your checking and credit card information will not be sold, shared or made available in any manner to any outside party except where there is an established and known business reason. Unless otherwise authorized in writing by the patient, protected health care information will only be used to provide treatment, to seek insurance payment or to perform other specific health care operations. Such information may be released to comply with a court order or other legal process.

As of April 14, 2003, our HIPAA privacy program is available on our website or from Fagen Pharmacy Privacy Officer. Prescription Medications
If you have any questions regarding your prescription(s) or feel you may have received something in error, please contact the pharmacist of the store where the prescription was purchased immediately.
Payment Policy
Payment of merchandise is due at the end of the sales transaction. Fagen Pharmacy accepts Discover, MasterCard and Visa.
